Claim Filing

When actual or anticipated payments reach 50% or more of the attachment point, please complete and send us an Early Notification Form, or other form or format designated by and approved by HCC Life that contains the same fields as the Early Notification Form.

The most efficient method of claim filing is electronically through the HCC Life eClaims Management Program.  Electronic claim filing utilizes our secure Web site so privacy is maintained.  You will save time, as well as paper and mailing costs, using eClaims.  To participate in the eClaims Management Program, please contact your HCC Life representative.

However, we do also accept paper claims. To submit claims manually, please:

1. Complete an Early Notification Form, or other form or format designated by and approved by HCC Life that contains the same fields as the Early Notification Form

2. Submit all supporting claim documentation. Mail or fax to:
