Eligibility
- Employee must have Optional Employee Life coverage
- If Employer chooses to offer this benefit, it is automatically included with any Optional Employee Life participant's benefit. Employees may not opt out of the coverage

- Loss must occur while insured and within 180 days of a covered accident

Additional Benefits Available (at the employer's option)
Seat Belt Benefit
Additional benefits may be available if a covered employee dies as a result of an auto accident for which Optional Employee AD&D benefits are payable and was wearing a properly fastened seat belt.
Air Bag Benefit
Additional benefits may be available if a covered employee dies as a result of an auto accident for which Optional Employee AD&D benefits are payable and was riding in a car with an approved, functioning air bag system for the employee's seating position.
Higher Education Benefit
If a covered employee dies as a result of a covered accident for which Optional Employee AD&D benefits are payable, additional benefits may be available to cover educational expenses for dependent children under 24 who are attending an accredited educational institution.
Common Carrier Benefit
Additional benefits may be available if a covered employee dies as a result of an accident for which Optional Employee AD&D benefits are payable and was riding as a fare-paying passenger on public transportation.
Surgical Reattachment
50% of the Optional Employee AD&D benefit that would otherwise have been payable for a covered employee's permanent loss of limb may be available if the limb is reattached.
Felonious Assault
An additional 25% of Optional Employee AD&D benefit that would otherwise be payable for a covered employee's loss may be available if the loss is a result of a felonious attack.
Repatriation Benefit
Additional benefits paid for transportation of an employee's body if death was covered under the Optional Employee AD&D benefit and occurred 50 miles or more from the employee's normal place of residence.
Child Care Benefit
If an employee dies as a result of an accident for which an Optional Employee AD&D benefit is payable, child care expenses up to a maximum of 5% of the Optional Employee AD&D benefit or $5000 per year to an overall maximum of $20,000 may be available.
Psychological Therapy Benefit
If an employee has a loss for which an Optional Employee AD&D benefit is payable, an additional 5% of the amount payable for that loss may be available for psychological therapy.
Rehabilitation Benefit
If an employee has a loss for which Optional Employee AD&D benefits are payable, additional benefits up to the amount payable for the loss or $10,000; whichever is less, may be available for expenses incurred for a qualified rehabilitation program.
Occupational Training Benefit
If an employee dies as a result of an accident for which Optional Employee AD&D benefits are payable, additional benefits up to $10,000 or the amount of Optional Employee AD&D benefits; whichever is less may be available for expenses incurred by the spouse of the deceased employee for participation in an approved occupational training program.
Family Transportation Benefit
If an employee has a loss for which Optional Employee AD&D benefits are payable and the employee is hospitalized more than 150 miles from the employee's normal place of residency, an additional $1,000 or the amount payable for the loss; whichever is less, may be available for the immediate family's accommodations near the hospital and transportation from the facility to the hospital.
Common Disaster Benefit
If an employee and eligible dependent spouse both die in the same accident for which an Optional Employee AD&D benefit is payable, an additional amount equal to the Optional Employee benefit or $500,000; whichever is less may be available. The spouse must also be covered for Optional Dependent AD&D under this policy. The benefit is only available if there are surviving eligible dependent children.
Health Insurance Continuation Benefit
If an employee dies as a result of an accident for which Optional Employee AD&D benefits are payable, additional benefits may be available as reimbursement for eligible dependents' group medical and/or dental insurance premiums.
Home and Vehicle Modification Benefit
If a covered employee suffers the loss of both feet, both legs or becomes a paraplegic, hemiplegic, or quadriplegic and requires the use of a wheelchair, an additional benefit of up to $10,000 or the amount of Optional Employee AD&D benefits payable; whichever is less, may be available for expenses incurred for alterations to the employee's residence or motor vehicle.
Coma Benefit
If a covered employee is in a coma as a result of an accident for which an Optional Employee AD&D benefit is payable and the coma has continued for at least 31 days, an additional annual benefit of 1% of the Optional Employee AD&D benefit that is payable for Loss of Life may be available for a maximum of 60 months.
Travel Accident Benefit
If a covered employee has a loss for which an Optional Employee AD&D benefit is payable and the loss occurs while traveling on company business for the policyholder, an additional amount equal to the Optional Employee AD&D benefit that would otherwise be payable for the loss or $500,000; whichever is less, may be available.
