Optional Dependent Life
Standard Eligibility
- Employee must have Optional Employee Life coverage
- Premiums are fully paid by the employee
- Benefits terminate when the employee retires or reaches age 75
Benefit Schedules
Incremental Benefit Amounts for Spouses:
- Increments of $5,000
- Maximum is the lesser of $100,000 or 50% of the employee amount
- Other options may be available
Spouse Age Reductions match Optional Employee Life
Spouse Guaranteed Issue Amount is $25,000 or 50% of the employee's benefit; whichever is less. Other options may be available.
Incremental Benefit Amounts for Children:
- $2,500, $5,000, $7,500, or $10,000
- All eligible children in a family have the same benefit
- All amounts are guaranteed issue
Increases after the initial enrollment require proof of good health.
Optional Spouse Life rates are the same as Optional Employee Life rates, but based on the spouse's age.
Optional Child Life rates are per $1,000. The rate is not charged per child.
Standard Provisions
Extension of Life Insurance if included under Optional Employee Life
Waiver of Premium is NOT available for Optional Dependent Life
Conversion Privilege
Portability if included under Optional Employee Life
Suicide Exclusion
Benefits are not paid for Optional Dependent Life if death is due to suicide during the first 2 years of coverage.
