Basic Dependent Life
Eligibility
- Employee must have Basic Life coverage
- Spouses
- Dependent Children from 14 days through age 18, or through age 24 if a full-time student
Benefit Schedules
Spouse/Child over 6 months/Child 14 days to 6 months
- Plan A $2,500/$1,000/$100
- Plan B $5,000/$2,500/$250
- Plan C $10,000/$5,000/$500
Standard Provisions
- Waiver of Premium or Extension of Life insurance - If the employee is totally disabled, Basic Dependent Life benefits may be continued without payment of premiums depending on the option provided under Basic Employee Life.
- Conversion Privilege - Dependents who lose coverage because they are no longer an eligible dependent or the employee terminates employment may be eligible to convert their lost benefits to an individual policy without providing evidence of insurability.
- Basic Dependent Life Rate - Basic Dependent Life rate is a family unit rate.
